Beneficiar(ies) for Individual Life Policy: If you wish to update your Beneficiary, please do so by logging in to the AICPA Insurance Trust Online Self Service Center at www.trustmyaccount.cpai.com and submitting online. First time users must register. Or, if you prefer, download, print and complete the Change of Beneficiary Form for Individual.

Change of Name or Assignment
Change of Name: If you have a name change print a blank Change of Name Form as a PDF file. Note: When completing this form, you will need to have the Name, Social Security Number and Account Number of the Insured.
Assignments: If you wish to change ownership and assign your account, download and print a blank Assignment Form as a PDF file. Note: When completing this form, you will need to have the Name, Social Security Number and Account Number of the Insured.
